Completing an on-site Health Screening is one of three wellness qualifications employees must complete in order to qualify for the Low Deductible Health Plan for FY19* or earn the maximum State contribution for your Health Savings Account (HSA), if you choose the High Deductible Health Plan for FY19.*
To register for the health screening, go to benefit.staywell.com, click on Let’s Go in the Health Screening block, then click on the link to register. You will need to verify your contact information and accept Quests Terms and Conditions in order to register.
New this year: You must register at least two weeks in advance to secure an on-site health screening appointment. Walk-ins will be accepted on a very limited basis.
Health screenings will be available in locations around South Dakota between August 1 - December 15, 2017. Find a location near you.
About the on-site health screening:
- Your 15-20 minute screening appointment will include the following measurements:
- Height, weight and waist
- Blood pressure
- Cholesterol (including HDL)
- If possible, fasting is recommended, but not required for accurate test results. Fasting includes only water (no black coffee or tea) for 9 to 12 hours before your appointment.
Alternatives to the on-site health screening:
If you missed the on-site health screening opportunity, you can still meet the screening qualification by visiting your health care provider and submitting the appropriate Health Care Provider Form at benefit.staywell.com. Members are eligible for one annual wellness preventive exam covered by the health plan each year. The cost of the screening tests will only be covered if it is part of an annual wellness preventive exam.
