To be eligible for the Latitude Health Plan (lowest deductible plan) in FY 2014, employees and covered spouses must complete a Health Screening AND Health Assessment.

Health Assessments must be completed between January 2, 2013 and March 31, 2013 to be eligible for the Latitude Health Plan in FY 2014 (Health Assessments completed before January 2, 2013 and after March 31, 2013 are not eligible).

By completing the Health Assessment, you receive an immediate picture of your health. Based on your responses, you receive a personalized report of your current health, with tips on how to prevent or reduce your individual health risks.

Newly hired employees should complete the Health Assessment within 60 days of your date of hire.

Your user name is comprised of the following:
Using your DAKOTACARE ID card, enter the Identification # (9 digits) + the 2-digit number before your name (written as 01, 02, etc.).

After completing the Health Assessment, you will receive a call from a HealthFitness health advisor who will share insight about your Health Assessment results. The Health Advising call is confidential, voluntary, and provided as a free service to members. For more information about Health Advising, click here.

Beginning in early January 2013 participants will see a Rewards Box appear on the Latitude Wellness website. Once a participant completes the Health Assessment, a check mark will appear within 48 next to the Health Assessment in the Rewards Box. This is a good way to confirm you have completed one of the necessary steps to earn the Latitude Health Plan.
