After completing the Health Assessment, you will receive a call from a HealthFitness health advisor who will share insight about your Health Assessment results.

During the consultation, the health advisor will:

• discuss the results of the Health Assessment and identify any areas for improvement,
• provide referral resources specific to your needs, and
• assist with questions about enrolling in the EMPOWERED Lifestyle Coaching program.

After you complete a Health Assessment and speak with a health advisor, you will be eligible for a confidential one-on-one lifestyle coaching program, EMPOWERED Lifestyle Coaching, to support you in making lifestyle changes to improve your health.
